Understanding your hormone labs.

Total versus free testosterone, why the blood draw has to be in the morning, what actually counts as "low," and what the rest of the panel is really measuring. This is the plain-English version: the same reasoning your doctor should be walking you through, minus the jargon and the sales pitch.

Why the labs come first.

Low testosterone has a real set of symptoms: fatigue, low libido, softer erections, low mood, brain fog, losing muscle despite training. The catch is that not one of those symptoms is specific to testosterone. Every single one of them is also a classic feature of poor sleep, an underactive thyroid, unmanaged stress, alcohol, certain medications, and depression. Low energy and weight gain feed each other, too. That's its own conversation. If you treated the symptom list without checking the blood, you'd be guessing, and half the time you'd guess wrong.

That's why every serious guideline says the same thing: a diagnosis of testosterone deficiency requires both consistent symptoms and unequivocally low testosterone measured on more than one morning.[1] The Endocrine Society and the American Urological Association are explicit that you do not treat a number without symptoms, and you do not treat symptoms without a confirmed low number.[1][2] Labs aren't a formality on the way to somewhere else. They're how you find out whether your hormones are your problem at all.

Total, free, and the protein in the middle.

When a lab reports your "testosterone," it usually means total testosterone: every bit of the hormone floating in your blood. But most of that isn't available to your body. The majority is stuck to a carrier protein called sex hormone–binding globulin (SHBG), with a bit more loosely attached to albumin. Only the small fraction that's unbound (your free testosterone) plus the loosely bound part is actually able to get into cells and do its job.

Think of SHBG as a set of parking spaces. When there are lots of spaces, more testosterone is parked and unavailable, and your total can look reassuring while the free portion runs low. SHBG isn't fixed, either: it rises with age and with an overactive thyroid or liver disease, and it falls with obesity, insulin resistance, and an underactive thyroid. So in the exact people who most often ask about testosterone (men carrying extra weight, men over 50), total testosterone can quietly mislead in either direction.

This is why a thoughtful workup doesn't stop at one number. When symptoms and total testosterone don't line up, or when something is likely throwing SHBG off, your doctor looks at free testosterone (measured by a reliable method or calculated from total testosterone, SHBG, and albumin) to see the fuller picture.[1] One value in isolation isn't a diagnosis. It's a starting point.

Why morning, and why twice.

Testosterone runs on a daily clock. In healthy younger men it peaks in the early morning and drifts down through the day, so a level drawn at 8 a.m. and one drawn at 4 p.m. can tell completely different stories about the same person.[3] Because the reference ranges labs use were built from morning samples, guidelines call for the blood to be drawn in the morning (generally before 10 a.m.), ideally when you're fasting.[1] An afternoon draw that comes back "low" may just be an afternoon draw.

The "twice" part matters just as much. Testosterone varies day to day for ordinary reasons: a poor night's sleep, a recent illness, a hard training block, even the assay the lab happens to run. A single low reading is not a diagnosis. That's why both the Endocrine Society and the AUA recommend confirming a low result with a second morning measurement on a separate day before anyone draws a conclusion.[1][2] Any clinic willing to settle your diagnosis from one convenient reading is skipping a step that exists specifically to keep you from being treated for a problem you don't have.

What "low" actually means.

There's no single magic cutoff, which is the part most marketing glosses over. The AUA describes a total testosterone below 300 ng/dL, confirmed on two morning draws, as a reasonable threshold to diagnose testosterone deficiency alongside symptoms.[2] But reference ranges differ between labs and between assay methods, the "normal" range is wide, and where you fall within it interacts with your SHBG, your age, and how you actually feel.[1]

So the honest rule cuts in two directions at once. You read the person, not just the number. A man sitting just above a cutoff with textbook symptoms deserves a real conversation, not a dismissive "you're in range." And a normal number is an answer, not an obstacle. A man who feels off but whose confirmed morning testosterone is solidly normal does not have testosterone deficiency, and calling it that anyway trades a fixable question (what's actually causing the symptoms?) for a dead end. A good practice is comfortable telling you the numbers don't point where you expected, and then helping you look in the right place instead.

The line that matters: a lab value without symptoms isn't a disease, and symptoms without a confirmed low value aren't testosterone deficiency. Responsible care needs both to point the same way. When they don't, good care says so plainly.

A responsible workup looks well beyond the testosterone value itself. Here's what else belongs on the panel, and what each number is actually measuring.

Total & free testosteroneThe headline numbers: total is everything in the blood, free is the active fraction. Read together, and always from a morning draw.
SHBGThe carrier protein that decides how much of your testosterone is available. Needed to make sense of a total that doesn't match your symptoms.
LH & FSHThe signals your brain sends to your testicles. They tell your doctor where a problem lives: high LH/FSH with low testosterone points to the testicles (primary), while low or normal LH/FSH points upstream to the pituitary or hypothalamus (secondary). That difference changes the workup and sometimes the whole plan.
EstradiolA form of estrogen men make from testosterone. It matters for bone, libido, and mood, and it's part of reading the hormone picture as a system rather than a single line.
Hematocrit (CBC)The share of your blood made up of red cells: in plain terms, how thick your blood is. Testosterone influences red cell production, so hematocrit helps place your level in context and is a standard safety marker to establish at baseline and follow over time.[1]
PSAA prostate marker, established at baseline in the appropriate age groups and followed over time so any change is caught and evaluated early.[1]
Prolactin, thyroid & metabolic labsOrdered when the story calls for it. These catch the pituitary, thyroid, and metabolic problems that masquerade as low testosterone, so you don't get treated for the wrong thing.

You will not need every one of these on day one, and your doctor will explain which apply to you and why. But a practice that measures one number, reaches a conclusion, and never looks again isn't evaluating you. It's guessing with a lab slip.
